First-Time & Renewal Forms

Application for a U.S. Passport
The standard form for first-time applicants, minors under 16, and anyone not eligible to use Form DS-82.
When to use
First-time passport applicants, children under 16, or if your last passport was issued before age 16 or more than 15 years ago.
U.S. Passport Renewal Application for Eligible Individuals
Mail-in renewal form for adults who already have a passport that is undamaged and was issued within the last 15 years.
When to use
You are 18 or older, have your most recent passport, it was issued within 15 years, and it is not damaged.
Large Book Passport Request
Standard DS-11 with the large book option selected for more visa pages.
When to use
You are a frequent international traveler and want the large 52-page passport book instead of the standard 28-page book at no extra cost.
Name Change & Data Correction

Application for a U.S. Passport - Name Change, Data Correction, and Limited Passport Book Replacement
Used to correct a printing error, change your name within one year of issuance, or replace a limited-validity passport.
When to use
Name changed within 1 year of passport issuance, data error on your passport, or replacing a limited-validity emergency passport.
Affidavit Regarding a Change of Name
Used to document a legal name change when court documentation is not available.
When to use
You have changed your name and cannot provide a court order or marriage certificate as proof.
Lost, Stolen & Replacement
What to file when your passport is lost, stolen, or never arrived in the mail.
Statement Regarding a Lost or Stolen Passport
Required form to report a lost or stolen passport book or card to the U.S. Department of State.
When to use
Immediately after discovering your passport has been lost or stolen. Must be submitted with Form DS-11 if replacing.
Statement of Non-Receipt of a Passport
Use this form if your newly issued passport never arrived in the mail.
When to use
Your passport was mailed but you did not receive it within 10 business days of the issue date shown in your status.
Minor / Child Passport Forms
Additional documentation required when applying for a passport on behalf of a child under 16.
Statement of Consent: Issuance of a U.S. Passport to a Minor Under Age 16
One parent or guardian can use this to give written permission for the other parent to apply for their child's passport.
When to use
One parent cannot be present at the acceptance facility and must provide notarized consent.
Statement of Exigent/Special Family Circumstances for Issuance of a U.S. Passport to a Minor Under Age 16
Used when one parent's consent cannot be obtained due to special circumstances.
When to use
The other parent's consent is unobtainable due to exigent circumstances or special family situations. Supporting documentation is required.
Statement of Consent for a Minor
Alternative consent document for minor passport applications.
When to use
When one parent or guardian needs to provide consent but cannot appear in person at the acceptance facility.

Birth Affidavit

When to use

Used when you do not have acceptable primary photo identification to present with your passport application.
When to use
You lack a driver's license, state ID, or other primary ID. A witness who has known you for at least 2 years and has valid ID must accompany you.
Identifying Witness Affidavit
Supporting affidavit for identifying a passport applicant without standard primary identification.
When to use
Secondary identification document when primary photo ID is not available. Witness must be a U.S. citizen or permanent resident.
Application for Consular Report of Birth Abroad
Used by U.S. citizen parents to document a child born abroad as a U.S. citizen.
When to use
Your child was born outside the United States and at least one parent is a U.S. citizen. Must be filed at a U.S. embassy or consulate.
